Therapeutic Botox® for Cervical Dystonia & Orofacial Dystonia
Involuntary muscle contractions affecting the neck, jaw, face, tongue, or lower face can significantly impact comfort, appearance, communication, eating, swallowing, and daily activities. Conditions such as cervical dystonia and orofacial dystonia are neurological movement disorders that can cause chronic pain, abnormal posturing, muscle spasms, and functional limitations.
At Alpenglow Pain & Wellness in Anchorage, Alaska, Nichelle C. Renk, MD, who is board-certified in anesthesiology and interventional pain management, and Gavin Nixon, DO, who is board-certified in Physical Medicine & Rehabilitation, provide comprehensive evaluation and treatment for dystonia and muscle spasm disorders. Therapeutic Botox® injections can help reduce abnormal muscle activity, improve function, and enhance quality of life for patients living with these challenging conditions.
What Is Dystonia?
Dystonia is a neurological movement disorder that causes involuntary muscle contractions, resulting in repetitive movements, abnormal postures, muscle spasms, and pain. Dystonia can affect a single body region or multiple muscle groups.
Two common forms treated with therapeutic Botox® include:
- Cervical dystonia (neck muscles)
- Orofacial dystonia (facial, jaw, mouth, and tongue muscles)
Symptoms may gradually worsen over time and can often interfere with daily activities, sleep, work, social interactions, and overall well-being.
What Is Cervical Dystonia?
Cervical dystonia, also known as spasmodic torticollis, affects the muscles of the neck and shoulders, causing involuntary contractions that pull the head into abnormal positions.
Symptoms may include:
- Neck pain and stiffness
- Head turning to one side
- Chin tilting upward or downward
- Head pulling forward or backward
- Muscle spasms and tightness
- Tremors of the head or neck
- Shoulder pain
- Chronic headaches related to muscle tension
- Reduced range of motion
Many patients experience significant discomfort and fatigue from the constant muscle contractions associated with cervical dystonia.
What Is Orofacial Dystonia?
Orofacial dystonia affects the muscles of the face, jaw, mouth, tongue, and lower facial structures. These involuntary muscle contractions may cause abnormal movements that interfere with speaking, chewing, swallowing, facial expression, and social interaction.
Orofacial dystonia may include:
Oromandibular Dystonia
Affects the jaw, tongue, and lower facial muscles and may cause:
- Jaw clenching
- Jaw opening or deviation
- Teeth grinding
- Difficulty chewing
- Difficulty speaking
- Tongue spasms
- Facial pain
Facial Dystonia
May affect muscles around the eyes, cheeks, mouth, or facial expressions and can cause:
- Involuntary facial movements
- Facial tightness
- Muscle spasms
- Difficulty controlling facial expressions
These conditions can be frustrating, painful, and socially limiting for many patients.
How Botox® Helps Treat Dystonia
Botox® is FDA-approved for the treatment of cervical dystonia and is widely utilized to treat various forms of focal dystonia affecting specific muscle groups.
Botox® works by temporarily blocking the nerve signals that trigger excessive muscle contractions. By relaxing overactive muscles, treatment may help:
- Reduce muscle spasms
- Improve posture and positioning
- Decrease pain and discomfort
- Improve range of motion
- Reduce headaches associated with muscle tension
- Improve speaking and swallowing function
- Improve chewing and jaw function
- Enhance quality of life and daily functioning
Treatment plans are individualized based on each patient's symptoms, anatomy, and affected muscle groups.
What to Expect During Treatment
Therapeutic Botox® injections are performed in-office and typically take less than 30 minutes.
During treatment:
- A comprehensive evaluation is performed to identify affected muscles
- Injection sites are customized to your condition
- Small amounts of Botox® are precisely administered into targeted muscle groups
- Ultrasound guidance may be used when appropriate to improve accuracy and safety
Most patients tolerate treatment well and are able to return to normal activities shortly afterward.
Results generally begin within several days to two weeks and may continue to improve over the following weeks. Benefits typically last approximately three months before repeat treatment is recommended.
